Scan a SMART® Health Card QR Code to verify an individual’s COVID-19 test or vaccination credential. Use the free Verifier app at your small business, live music venue, school, or sporting arena.
The SMART® Health Card Verifier app allows you to scan a SMART Health Card QR code to quickly verify an individual’s COVID-19 test or vaccination credential. Scanning a SMART Health Card QR code:
Verifies whether the SMART Health Card is valid (i.e., not tampered with).
Confirms the SMART Health Card was issued by a participant in The CommonTrust Network’s Registry of trusted issuers.
Displays key information on the SMART Health Card (issuer name, vaccine or test type, dates of vaccine doses or test, and name and date of birth of recipient).
Use the camera within the app to scan SMART Health Card QR codes and one of the three results will appear.
If a SMART Health Card is verified: The SMART Health Card scanned comes from a verified issuer in The CommonTrust Network’s registry of trusted issuers and was not tampered with.
If a SMART Health Card issuer is not recognized: The SMART Health Card scanned may be valid, but the issuer is not in the CommonTrust Network’s registry of trusted issuers. The issuer URL will be provided.
If a SMART Health Card is not verified: The SMART Health Card scanned is not valid, as it may have been corrupted, or modified after it was issued.
Please note: The Verifier app only scans SMART Health Cards. It does not scan paper CDC cards or other types of health credentials. The SMART Health Card Verifier App does not store SMART Health Cards.
Download the app and you’re ready to go. The app does not store any personal data about you as the user of the app.
Only key information needed for verification is shown when a SMART Health Card is scanned, and no personally identifiable information from the cards you scan will be stored.
The SMART® Health Card Verifier app is a free service brought to you by The Commons Project Foundation, built on standards from SMART Health IT.
